"Take Your Shot” Incentive Campaign
Terms and Conditions
1. The “Take Your Shot” Incentive Campaign Lucky Draw (“The Lucky Draw”) is organised by The Chinese Manufacturers’ Association of Hong Kong (“CMA”).
2. The Lucky Draw campaign runs from 18 July to 29 September (“Campaign Period”).
Eligible Participants
3. Participants of the Lucky Draw must be a holder of a valid Hong Kong Identity Card (Permanent Resident or Non-Permanent Resident) who is at least 18 years old and who has already received the full two-dose course of a COVID-19 vaccine in Hong Kong on the day of Lucky Draw registration. (Persons who satisfy the first two conditions and were infected with COVID-19 will be eligible after they have received one dose of a COVID-19 vaccine.)

4. Two rounds of the Lucky Draw will be held during the campaign period:

Registration for the Lucky Draw
6. From 18 July, eligible participants can register for the Lucky Draw on a designated application or system (“MyeHealthPass”). Participants need to complete their registration by entering their information and agreeing to all the Terms and Conditions. Once registered, participants can join the Lucky Draw and view the latest information about the campaign. Participants’ data is encrypted and protected by blockchain technology.
7. Once registered, participants can log into their “MyeHealthPass”account to track the progress of the Lucky Draw and the status of their participation.
8. Participants need to register for the Lucky Draw only once. Those who register for the First Draw will automatically be entered into the Grand Draw.
9. Participants can amend their personal data (except their email address) or cancel their registration with “My eHealthPass” at any time.
Earn extra Lucky Draw chances by spending at designated merchant outlets
10. Each participant will have only one chance to win a free spending credit prize over the period of the campaign. However, a participant can enjoy an additional chance of winning a non-spending credit prize in the Grand Draw by spending at least $100 (single purchase) at designated merchant outlets (Allied Merchants) during the period of 18 July and 18 September. Participants can enjoy up to five additional chances of winning non-spending credit prizes by making purchases provided that each purchase is made at different merchants. In order to be eligible for said additional chances, registered participants are required to input the spending information into the “MyeHealthPass”.
11. Participants need to retain the original receipt for verification. Only payment receipts issued by Allied Merchants can be used for Lucky Draw entries. The purchase information provided by participants must be exactly the same as the information indicated on the receipt. Information could not be altered once it is submitted. Handwritten receipts should include reference number and must be stamped. Combined payment receipts will not be accepted. Each receipt can only be used to enter into the Lucky Draw once.
Results Announcement
12. Winners will be drawn randomly. The results of the two draws will be announced within 10 working days on the CMA website after each draw. The results of major prizes will be published in Sing Tao Daily and The Standard on 5 October 2021. All winners will be notified individually via email. Winners can also view their Lucky Draw result via“MyeHealthPass”.
Collection of Prizes
13. The collection arrangements for Lucky Draw prizes (“Prizes”) will be notified to participants via email. All winners must agree the terms and conditions of the Prizes. Winners of a free spending credit prize and major prizes must redeem the Prize in person. Winners of other prizes can delegate others to redeem the Prize. The winner or delegate who fails to collect their Prizes on the date or time or from the venue specified in the collection notice shall be deemed to have waived the right to claim their Prizes. All winners/ delegates shall bring along the necessary identification document, the original copy of the COVID-19 vaccination records issued by the Department of Health of the Hong Kong Government, delegation of authority letter (if applicable) and payment receipts (if applicable) to collect the Prizes.
Personal Data
14. The provision of personal data is entirely voluntary. However, if a participant does not provide “MyeHealthPass”with the requested data, he/she may not participate in the Lucky Draw. The personal data collected in the course of the Lucky Draw will be collected only for the use of the Lucky Draw which shall include verifying the participants’eligibility and identity, announcements of Winners, contacting Winners on Prize collection arrangements and all directly related purposes. The personal data may be disclosed and transferred to the Lucky Draw prize suppliers, and respective service providers for the purposes in relating to the Lucky draw, and on a need-to-know basis, without the relevant participant’s prior consent. By registering with “MyeHealthPass” and participating in the Lucky Draw, and by providing the personal data, the participants agree to the collection and understand the purpose of the collection, and accept all terms and conditions of “MyeHealthPass” and the Lucky Draw. The personal data provided by participants will be properly stored and encrypted, and the CMA will destroy the data within 90 days after the end of the Lucky Draw campaign period.
For withdrawal from the Lucky Draw, please send an email to the CMA (info@cma.org.hk) . If you want to cancel your registration with “MyeHealthPass”, you can delete your account directly on the application.
General Terms and Conditions
15. All participation in the Lucky Draw is voluntary and the CMA and the prize sponsors shall not be liable for any damages, losses, adverse health effects, injury, claims, costs or proceedings incurred or suffered by participants as a result of their participation of the Lucky Draw.
16. Prizes are not resalable, returnable nor redeemable for cash or other gift items. Any unused Prizes, services and related entitlements will be cancelled after their validity date expires and without any prior notice or compensation. Prizes are not replaceable in the event of loss or damage.
17. Use of the Prize may be subject to the prize suppliers’terms and conditions applicable thereto. The CMA is not responsible for and shall have no liability in respect of the quality of goods and services provided by the suppliers of the Prize.
18. The CMA has absolute discretion in according to the relevant clauses in this Terms and Conditions, determining a person’s eligibility to receive a Prize. The CMA is entitled to disqualify a person from participating in the Lucky Draw and receiving a Prize if he or she has failed to comply with these Terms and Conditions. The CMA shall not have any legal liability from judging a participant disqualified and the participant cannot object to such disqualification.
19. In the event of any dispute arising from the Lucky Draw, the CMA’s decision shall be final and conclusive.
20. All information provided by participants to the “MyeHealthPass” shall be treated as final and definitive. Any duplicated, faulty and incomplete information will not be considered valid. If participants are unable to receive notification of Prize collection due to any fake, incomplete or incorrect information provided, or due to any email delivery failure or inappropriate app notification setting, the CMA shall bear no responsibility.
21. The CMA shall not bear any liability if the registered data of participants is lost, inaccurate, unidentifiable or damaged due to any network, telephone or technical issue.
22. The CMA shall not be held liable for any delay, disruption or non-availability of the “MyeHealthPass”.
23. Prize pictures in promotional materials are for reference only, the actual Prizes shall be considered as final.

25. The CMA reserves the right to make the final decision in the event of any dispute.
26. Should there be any discrepancy between the English version and the Chinese version of these terms and conditions, the Chinese version shall prevail.
